Understanding how a bicycle fork is assembled
A bike's fork includes a split bearing as shown in the picture to the right, which first half fits underneath the bike's frame head tube, and the other half above. Pay well attention to the parts that were above / below the frame when dismantling a fork, nevertheless, it's not difficult to identify these parts and refit them properly:

Re-assembling a bike's fork to the frame
Generously apply grease to the bearing and apply a light coating to the headset tube before inserting the fork into the frame.
Refit:
Then refit the upper washer with its bolt that holds all above parts together.
Tighten well its bolt after you make sure the handlebar is well oriented i.e. perpendicular to the frame.
